
A Victorian coroner has found that two off-road racing competitors who died in a 2022 crash survived the initial impact but were killed when fire rapidly engulfed their vehicle—deaths that could've been prevented with equipment now required but not mandated at the time.
Gerald Hoekstra, 46, and Ede Taric, 50, died on September 4, 2022, during the Desert Enduro Race at Rainbow, about 400 kilometres north-west of Melbourne. Coroner Ingrid Giles concluded the men's deaths were "unspeakably tragic" and directly linked to regulatory gaps that allowed a fuel system vulnerable to catastrophic failure.
The Fatal Sequence
Mr Hoekstra lost control of their modified Can-Am Maverick X3 side-by-side buggy and crashed into a tree. The vehicle rolled and landed on its side. Competitor teams stopped immediately and saw Mr Hoekstra attempting to exit. They couldn't extinguish the flames with fire extinguishers. Both men died at the scene before the Country Fire Authority arrived 12 to 13 minutes later.
Victoria Police arson chemist John Kelleher determined the fire started when heat from the buggy's catalytic converter ignited fuel that spilled after the crash. "Having seen the buggy and watched the video footage of the fire starting, there is no doubt that the cause of the fire was the ignition of petrol vapour rising from the spilled fuel puddling beneath the filler," he said. The catalytic converter's position immediately above the spilled fuel made it the likely ignition source.
Regulatory Compliance Wasn't Enough
The buggy had been inspected by a scrutineer the Friday before racing and met all Motorsport Australia requirements. It carried hand-held fire extinguishers and two auxiliary 35-litre fuel tanks capable of propelling the vehicle to speeds of 130km/h. But compliance with existing standards didn't prevent the tragedy.
Ms Giles found the fuel tanks had plastic fuel caps and filler necks that protruded from the buggy's safety cage, leaving them "vulnerable to damage." Upon collision, the cap detached from the filler neck on one tank, which lacked a reflux valve. "I consider that, had a reflux valve been present, the fire would have most likely been prevented, in that the fuel would not have spilled freely from the tank," she said.
The hand-held fire extinguishers required by Motorsport Australia regulations were "inadequate" because the fire couldn't be safely extinguished without CFA assistance. A plumbed-in fire extinguisher system would've been more effective, Ms Giles said, providing "an immediate extinguisher response that can increase the amount of time available to extricate from the vehicle during an incident."
Industry Response and Remaining Gaps
Ms Giles credited Motorsport Australia for making changes after the deaths, saying its decisive action had "obviated the need for an extensive suite of coronial recommendations." Since the incident, MSA has mandated reflux valves in fuel tanks, banned structures protruding outside safety cages and doubled the required capacity for hand-held fire extinguishers.
But the coroner expressed concern that MSA's three-year roll-out period for mandatory plumbed-in fire extinguisher systems puts competitors at unnecessary risk. "The lives of off-road racing competitors who do not already have such a system remain at greater risk in the event of a fire—and, in my view, unnecessarily so," she said.
She recommended that before the end of the 2026 racing season, Motorsport Australia and the Australian Auto-Sport Alliance require all off-road vehicles in sanctioned events to have plumbed-in fire extinguisher systems. She also called for mandatory testing to ensure each occupant can self-extricate within 10 seconds—a requirement that existed on paper but wasn't formally assessed at the time of the crash.

The men died not because they violated rules, but because the rules themselves were insufficient. Motorsport Australia's post-incident reforms show the industry can respond to tragedy, but the coroner's criticism of the extended implementation timeline raises questions about whether self-regulation moves fast enough to protect participants. The recommendation for mandatory extrication testing addresses a fundamental accountability gap: requiring something on paper means nothing if no one verifies compliance. For families who accept the inherent risks of motorsport, the distinction between unavoidable accidents and preventable deaths caused by regulatory inadequacy matters enormously. The coroner's findings suggest these deaths fell into the latter category.
