Obesity's Cancer Link: A Crisis for Europe's Working Class

A new study published today reveals that being overweight or obese could be responsible for more cancer cases than previously known, with abdominal fat particularly increasing the risk of cancer, especially among women. This finding exposes a deepening public health crisis, one that disproportionately impacts Europe's working class and reveals the structural inequalities inherent in a neoliberal system that prioritizes profit over collective well-being.

The research, led by Hermann Brenner, an epidemiologist with the German Cancer Research Center, suggests a stark reality: up to 11.5% of cancers may be attributable to being overweight. This figure marks a significant increase from previous estimates, which placed the contribution of excess body weight to cancer cases between 2% and 8%. Michael Leitzmann, an epidemiologist at the University of Regensburg, who was not involved in the study, underscored the gravity of these updated findings. He stated that the DKFZ researchers "have not only updated existing risk estimates, but have also questioned whether the link between excess weight and cancer may have been underestimated until now." This underestimation points to a systemic failure to grasp the full scope of public health challenges.

The Systemic Roots of Disease

The study changed its methods from earlier work that relied on body mass index, or BMI, alone. BMI doesn't distinguish between fat and muscle mass, nor does it show how fat is distributed. Leitzmann explained that fat around the abdomen isn't just a passive store of excess calories. Instead, he noted, "They trigger what is known as a chronic inflammatory metabolic state." He added that excess abdominal fat functions like a gland, releasing signaling molecules known as adipokines. These molecules travel through the bloodstream and influence cell growth, cell division, and the inhibition of programmed cell death. "These are all mechanisms that can contribute to the development and subsequent growth of tumors," Leitzmann confirmed, highlighting the complex biological pathways at play.

The DKFZ researchers focused not only on BMI but also on waist circumference and the waist-to-hip ratio. They used extensive data from 458,543 women and men from the UK Biobank, one of the world’s biggest health datasets. Participants were followed for an average of 12 years, during which 50,000 new cases of cancer were identified. The researchers also tried to rule out a bias in previous studies by excluding the first few years after enrollment. Only cancer cases that occurred seven years after the start of the observation period were considered, making the statistical link between being overweight and cancer clearer.

Prevention and Class Disparity

The study suggests that the proportion of overweight women who develop cancer is higher than that of men. Leitzmann urged caution, however, stating that further studies with a similar design are needed to see whether the results can be replicated. He noted that being overweight is blamed, among other things, for breast, uterine, and ovarian cancers, which are hormone-dependent. Estrogen can stimulate the growth of breast cancer, and excess abdominal fat can contribute to elevated estrogen levels, he explained. This gendered disparity in health outcomes further exposes the unequal burdens within society.

Leitzmann also pointed out that most people are now aware that obesity increases the risk of cardiovascular disease or Type 2 diabetes. He added that losing weight also reduces the risk of heart attacks, strokes, and diabetes. "To me, this clearly indicates that, by preventing obesity, we will be able to make a greater contribution to cancer prevention than previously assumed," he concluded. This emphasis on prevention underscores a critical public health responsibility, one that demands systemic action and equitable access to resources, rather than individual blame, especially for the working class who often face the steepest barriers to healthy living conditions.
